The sheath is well made and fits the knife. Like the others said, it doesn't hold the knife well right from the factory. I think everyone's MKM knife is slightly different, so I think settings your expectations properly is important: you can't make a perfect factory fit for hand finished goods.

Also, my experience with ALL Kydex products like sheaths: they take a little bit of heat treatment at home to get the final fit to your liking.

After a few blasts with the heat gun and some trial/error, my tpm now clicks in with a firm snap and breaks free with a satisfying click as well, and it no longer rattles while moving around. There are some good knife making videos on youtube about making kydex sheaths so I suggest taking a peek.

For my particular sheath, I need to "pinch" the bottom a bit, as well as push the "top" of the sheath in a hair. If you look at the tpm, you'll see that it has a protrusion on top of the knife for a thumb rest. Pushing down the top of the sheath a hair makes the holster contact the thumb rest protrusion. The click is achieved by getting the pinch correct on the bottom of the sheath.

The sheath fitting is not secure enough for scout (horizontal) carry, though heat, skill and patience can adjust it. The knife rattled a bit in this sheath, but it works well for pocket carry and is easy to thumb off. Personally, I do pocket carry with an UltiClip.
